Requirements For Submissions Of Applications To Distribute Banked Assessment Work Credits

• One copy of the Application to Distribute Banked Assessment Work Credits (form # 0291).

NOTE: Application must be filled out on separate forms if using more than one work declaration number.

• Ensure all sections of the form are completed, signed and dated. (best to obtain a copy of the mining claim reserve summary to indicate the proper work declaration number)
• Include a legible contiguity sketch which identifies the contiguous link between the claims (a photocopied portion of an up-to-date claim map is preferred).

NOTE: Claims where the work is being assigned must be contiguous to the claim(s) where the work was performed and must be on record at the time the original work was performed and reported.

• Provide an agent authorization letter if distributing credits on behalf of someone else if one has not already been submitted.
• Provide proof of beneficial interest or as having an option agreement on the property if someone else's claim is included on your application (if applicable).
