Revised Mining Act Forms

Effective August 8, 2006 the new Forms Regulation 195/06 (http://www.e-laws.gov.on.ca/DBLaws/Regs/English/060195_e.htm) comes into effect. The current prescribed forms and Minister's Approved Forms will not be legal forms as of August 8th.

The following forms are revised as of August 8th.

• Application to Record a Staked Mining Claim
• Assessment Work Performed Before Recording a Mining Claim
• Assessment Work Performed on Mining Lands
• Notice of Intention to Perform Assessment Work
• Notice of Re-staking of Transferred Claim
• Transfer of an Unpatented Mining Claim

Other forms might have minor changes.

All Mining Act forms currently located on the Mining Lands web page will be reformatted and re-located to the Government's Forms Repository web site on August 8.

You will be able to search by form number, form name or name of the Ministry. The forms will be available in PDF and HTML formats.

Mining Lands Section will keep a list of form names and numbers on its web page with a link to the Government Forms Repository.

Discard all unused old forms on August 8 and print the new forms.
