Mining Act Forms
The Mining Act forms listed below are now located on the Ontario Government Forms Repository site.
The only form not on the Government Forms Repository site is the OFSC Trail Permit Exemption (available in Word and PDF formats).
After completing a form on-screen you must
print it, sign it and then submit the paper copy. Note, the final
appearance and quality of a form is printer dependent.
